Well not "Everybody" smokes in the middle east but statistically smokers percentage in middle east is higher compared to western countries. However you can find lot higher smoking percentages in south asian countries.
I can see few reasons for this;
1- Education level of those countries. Bad habits, such as cigarettes, drugs and other addictions are commonly occur in lower educational societies.
2- Economic level. Societies with lower financial standarts usually are more willing to show weaknesses to bad habits.
3- Cultural difference. There are cultures in the world which believes strongly in the natures healing power. Therefore "Nature cant (wont) harm us. Nothing natural will harm a human." principle overcomes the smoking habit.
4- Level of religious beliefs. If the majority of the society is strong on their religion, usually they take actions with the idea of "Everything comes from God. It is just my destiny. If God will take my life, he will find a reason anyways.". This doesnt concern only Islam but most religions are pretty similar about this.
